If students have other qualifications other than HKDSE and / or Music Quaifications, you may need to submit certified copies to the JUPAS office. For details, you may refer to :

The school administration period for the certification procedure is : 15 Jan 2013 - 31 Jan 2013
Certification Procedure :

Fill in the C10 form from office.
1. Submit the original documents together with the photocopies (must be in the same order) to the school office.
2. The office will return the original certificates to you immediately after checking.
3. The certified copies will be available after being signed by the principal with school chop.
4. You should then mail the certified documents to the JUPAS office directly, with your application number at the upper right corner as stated clearly on web.
